Please fill out the form below if your student that will be attending Dixon Middle School’s Summer Program has any allergies or health needs that we need to be aware of. Just a reminder that medications have to be brought to the school by parent for our health clerk or nurse to give out (students cannot be in possession of medications; unless there is an order from the doctor ie. inhalers and epipen) We can give out Tylenol and Motrin at school with a call or text home for permission each time.

Solo un recordatorio de que los padres deben traer los medicamentos a la escuela para que nuestro asistente de salud o enfermero los entregue (los estudiantes no pueden estar en posesión de medicamentos; a menos que haya una orden del médico, es decir, inhaladores y epipen).
